Cat Head Tilt and Flickering Eyes: Vestibular Disease vs. the Brain
๐จ Check first
If, in addition to a head tilt and wobbling, your cat has seizures, seems dazed and does not respond when called, or has no strength at all in the legs on one side, the cause may not be deep in the ear but an emergency such as a stroke (cerebral infarction or hemorrhage) or a brain tumor. See a vet immediately.
๐ The answer in 3 seconds
- When a cat that walked normally yesterday suddenly holds the head tilted far to one side (head tilt), staggers and falls over like a drunk when standing up, has eyes that flick rapidly side to side or up and down like a metronome (nystagmus), or circles in the same direction, these are symptoms of vestibular disease, a problem in the organs that control balance.
- Common causes are inflammation deep in the ear (middle ear infection, inner ear infection, polyps) and idiopathic vestibular disease, which strikes senior cats suddenly for no known reason. In the latter case, with appropriate fluids and anti-nausea treatment, cats often improve within a few days to a few weeks.
- Your cat is panicking from sudden, violent dizziness and motion sickness, as if the world were spinning at high speed, so keep them resting in a small space with no steps or ledges.
๐ Flowchart
- Step 1First, take a smartphone video of how the head is tilted and how the eyes are moving (side to side, up and down, or rotating) (the movement may stop by the time you reach the clinic, so the video is important evidence for diagnosis).
- Step 2To prevent falls from a cat tree or sofa because of severe dizziness, move your cat right away into a single-level crate with no ledges (or a small space walled in with cushions), and drape a cloth over the top to dim it and reduce visual stimulation.
- Step 3Your cat is severely motion sick and nauseous, and lowering the face to the bowl makes them fall over, so do not force food. See a vet the same day for anti-nausea medication (antiemetics) and fluids.
โญ Step-by-step fix
Step 1 ยท What you can do at home right now
First, take a smartphone video of how the head is tilted and how the eyes are moving (side to side, up and down, or rotating) (the movement may stop by the time you reach the clinic, so the video is important evidence for diagnosis).
Step 2
To prevent falls from a cat tree or sofa because of severe dizziness, move your cat right away into a single-level crate with no ledges (or a small space walled in with cushions), and drape a cloth over the top to dim it and reduce visual stimulation.
Step 3
Your cat is severely motion sick and nauseous, and lowering the face to the bowl makes them fall over, so do not force food. See a vet the same day for anti-nausea medication (antiemetics) and fluids.
โ Never do this
- Leaving a wobbly cat that cannot walk on a high bed or sofa and looking away (they can fall head first to the floor and break a bone or get a concussion).
- Thinking the cause may be deep in the ear and pouring store-bought ear cleaner into the ear or pushing a cotton swab in at home (with a middle or inner ear infection where the eardrum has ruptured, putting liquid in can rapidly worsen nerve damage).
